In-Home NP/PA Clinician (Part-Time)

Signify Health, part of CVS Health, seeks a Part-Time Clinician (NP/PA) to perform in-home health evaluations for plan members in their homes. Evaluations include medication and history review, a physical exam, and coordination with the health plan for necessary tests, with no prescribing or regimen changes.

You will travel locally within licensed states, conduct virtual visits as needed, and join a national clinician network focused on improving health outcomes.
