Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations.
You will visit members in their homes for about 45 minutes, review medications and medical history, perform a physical evaluation, and coordinate with health plans. You may travel locally across licensed states and conduct virtual visits when needed.
