In-Home NP/PA Clinician — Flexible Per Diem Visits

Signify Health, part of CVS Health, seeks Per Diem Clinicians (NPs or PAs) to perform in-home health evaluations. Clinicians will visit members in their homes, review medical histories, perform physicals, and offer health education. Travel locally and some virtual visits may be required.

The role is part-time with a base hourly rate and incentive structure per visit, plus malpractice coverage and supplies. Benefit programs apply based on hours worked.
