In-Home NP: Flexible Part-Time Health Evaluator

Signify Health, part of CVS Health, seeks a Part Time Clinician (NP or PA) to perform in-home health evaluations in members’ homes across participating states. The role offers flexible local travel, approx. 29.5 hours per week, and a pay range of $46.03–$99.14 per hour. Board-certified NP or PA with active licenses is required.

You’ll assess health status, review medication and history, and support care coordination with health plans, while avoiding changes to treatment unless ordered.
