In-Home Health Risk Assessor (NP/PA) – Flexible Schedule

Hueman is hiring Nurse Practitioners and Physician Assistants to perform in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Linneus, ME. This role offers a flexible schedule and the freedom to work outside a traditional clinic, with travel to patients’ homes and support to document findings accurately.

Candidates will review medical history, perform non-invasive assessments, and provide basic wellness education.
