In-Home Health - Nurse Practitioner or Physician Assistant (Per Diem) - Pullman WA

Per Diem Clinician role for In-Home Health Evaluations with Signify Health, part of CVS Health. Travel locally as needed; virtual visits possible.

Responsibilities

• Provide In-Home Health Evaluations for individuals in their homes using an iPad and clinical workflow.
• Conduct medication and medical history review, physical evaluation, and, when ordered by the health plan, additional diagnostic tests.
• Build trusted relationships with members to connect them with the right care.
• Visit millions of people nationwide as part of a network to improve health outcomes.
• Provide Signify Health services including Diagnostic and Preventive Services (e.g., spirometry, bone density screening) as indicated.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as required.

Qualifications

• Active, unrestricted license in required coverage area(s).
• Board certification (required).
• Multistate licenses (preferred).
• Able to cover Pullman, WA primary location (required).

Per Diem Details

• Hourly base rate plus incentive structure per visit; malpractice insurance during Signify Health activities; supplies and other perks.
• Anticipated weekly hours: 12; Time Type: Part time.

Pay and Benefits

• The typical pay range: $46.03 - $99.14 per hour.
