In-Home Health NP/PA Clinician (Part-Time)

Signify Health in the United States seeks a Part-time Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional tests; prescribing is not required.
