Signify Health, part of CVS Health, is seeking a Full-Time Clinician (Nurse Practitioner or Physician Assistant) to perform In-Home Health Evaluations for members in the comfort of their homes. You will bridge gaps in care with a compassionate, patient-centered approach.
You will travel locally and within licensed states, conduct evaluations, review meds/history, and may perform diagnostic tests as ordered by health plans. Some virtual visits may also occur.
