In-Home Health Evaluator - NP/PA (Part-Time)

Signify Health, part of CVS Health, seeks a Part Time Clinician (NP or PA) to perform in-home health evaluations, visiting members in their homes and bridging gaps in care. Activities include medical history reviews and focused physicals, with optional diagnostic tests as directed by health plans.

You will work in a national network, travel locally within licensed states, and may conduct virtual visits as needed.
