Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. You will engage with people in their homes, bridging gaps in care and guiding them to the right services.
Visit patients for about 45 minutes, review medications/history, perform a physical evaluation, and order tests if their plan requires. Travel locally and within licensed states as needed, using an iPad with a clinical workflow.
