In-Home Clinician (NP/PA) – Part Time

Signify Health, part of CVS Health, is seeking a Part Time Clinician (NP or PA) to provide in-home health evaluations. You will engage with patients in the comfort of their own homes, performing reviews of medication history, physical assessments, and care coordination to bridge gaps in care.

Responsibilities include visiting patients nationwide within licensed states, completing virtual visits as needed, and delivering diagnostic and preventive services as indicated.
