In-Home & Telehealth Nurse Practitioner — Impactful Primary Care

HarmonyCares is seeking a Nurse Practitioner to perform in-home health risk assessments and telehealth visits within the NP scope of practice, as delegated by the Collaborative Physician.

You will conduct comprehensive evaluations, document visits to NCQA standards, take vital signs, arrange referrals, and coordinate with PCPs, patients, caregivers, and providers to ensure accurate diagnoses and quality care.
