To support the SNFist Team, the full-time Illinois Licensed SNF Resource Nurse will manage patient telephone interactions, triage calls for new admissions, and serve as a liaison among hospitalists, providers, and nursing homes while working remotely during daytime hours. Key responsibilities Complete necessary telephone communications with patients, families, and healthcare facilities to gather and maintain patient information Prepare "pre-charting" information for SNFist providers, including medication reconciliation and summaries of pertinent medical information Collaborate closely with SNFist Team nurses, providers, and nursing home staff to ensure timely patient admissions and continuity of care Required qualifications Licensed Registered Professional Nurse (RN) in Illinois Basic Life Support (BLS) certification within 30 days of hire from the American Heart Association (AHA) Bachelor's Degree in Nursing or related field Experience in nursing or healthcare settings Professional telephone etiquette and strong communication skills
