Facilitating patient transfers into and out of Carle Hospital, the part-time Illinois Licensed RN Specialist will manage communication between referring and accepting physicians, ensure appropriate patient placement, and oversee the transfer process while working remotely on a day shift schedule. Key responsibilities Expedite consultations between referring patient-care providers and receiving physicians to ensure timely and appropriate patient transfers Utilize established protocols to coordinate acceptance, admission, and transfer processes while maintaining clear communication with all parties involved Document all transfer-related information accurately in electronic medical records and provide follow-up communication to ensure continuity of care Required qualifications Licensed Registered Professional Nurse (RN) in Illinois Basic Life Support (BLS) certification within 30 days of hire College Diploma in Nursing Minimum of 1 year of experience with electronic medical records (EMR) Strong clinical assessment skills to evaluate patient acuity and needs
