To support kidney health management, the remote Illinois Licensed RN Care Navigator will manage a caseload of members, conducting health screenings, creating care plans, and collaborating with healthcare teams to ensure positive patient outcomes. Key responsibilities Handle inbound and outbound calls, providing exceptional service to members Educate members on kidney health, dialysis, and transplant options while engaging them in the Kidney Health Program Coordinate care as a liaison among patients, their support networks, and healthcare providers to resolve barriers and meet individualized goals Required qualifications Active, unrestricted RN license in Illinois and a compact state Bachelor's degree or 6+ years of RN experience, including 3+ years in case management CCM certification preferred Three years of experience in case management preferred Experience in a dialysis or transplant center preferred
