To provide high-quality patient care services, the Licensed Practical Nurse (LPN) will remotely monitor patients' health, engage with them through various communication methods, and collaborate with healthcare providers, with opportunities available for both part-time and full-time positions. Key responsibilities Utilize technology to remotely monitor patients' vital signs and provide necessary interventions based on data Engage with patients via phone, text, or video to remind them to record vitals and respond to alerts related to their health Collaborate with healthcare providers to develop care plans and educate patients on using remote monitoring technology Required qualifications Valid Licensed Practical Nurse (LPN) license in the state of Illinois Experience in a healthcare setting, preferably in remote patient monitoring or telehealth Ability to work independently and as part of a team Strong ethics and values, with critical thinking and problem-solving skills BLS certification required
