To improve health quality outcomes, the part-time Illinois Licensed Clinical Pharmacist will perform telephonic outreach to UHC Medicare members while working remotely with occasional office visits in Schaumburg, IL. Key responsibilities: Perform telephonic outreach to enhance medication adherence and appropriate drug utilization Optimize pharmacy therapy through engagement with providers and pharmacies Document and track member engagement to improve CAHPs member experience results Required qualifications: Bachelor's degree (or higher) in Pharmacy Current and unrestricted Pharmacist license in the state of Illinois 3+ years of experience working as a licensed pharmacist 3+ years of patient consultation experience while working as a licensed pharmacist Beginner level of proficiency navigating MS Office and Windows-based environment
