Illinois Licensed Care Navigator

To support patient engagement and health outcomes, the remote Illinois Licensed Care Navigator will manage physiological markers, conduct wellness calls, and collaborate with care teams to promote health behavior changes during CST hours. Key responsibilities Manage physiological markers such as blood pressure, weight, and blood glucose with clinical appropriateness Provide preventive health education and coaching through monthly wellness calls with patients Collaborate with care teams to define and achieve patient-centric health goals Required qualifications Unrestricted RN/LPN/LVN license in a compact state, preferably Illinois 1-3 years of clinical experience in patient or disease management Experience with remote patient monitoring (RPM) and telephonic care management Ability to work independently and manage patient communications effectively Spanish speaking is a plus
