Responsibilities:
- Provides case management services in the community to individuals with intellectual and developmental disabilities and their families
- Facilitates linking, advocating, coordinating, and monitoring services; to assist consumers in gaining access to health services, financial assistance, employment, education, social services, and natural supports
- Assesses needs, develops, and monitors Person Centered Plans and documents treatment
Qualifications:
- Bachelor in Social Work or Master's Degree in an appropriate mental health discipline
- QIDP, one years' experience providing treatment to individuals with Intellectual and Developmental Disabilities
- Must have reliable means of transportation for home visits and maintain Safe Driving Permit
