Perform telephonic care management services to assigned population
Act as the sole communicator between member and Plan
Review person-centered service plan with member/members family and coordination of covered and non-covered benefits
.Perform monthly contact and follow up with the members
Follow up and respond to identified items requiring attention
Participates in weekly team meeting
Background:
MSW, Masters in Social Work
LMSW is preferred
2-3 years of care management experience in any setting
Remote working/work at home options are available for this role.
