Housing/Case Manager
Under the supervision of the Clinical Supervisor, the Housing/Case Manager will provide case management services to a caseload and provide housing assistance within the facility.
Key essential functions include:
- Case Management:
- Provide case management services to a caseload of up to 15
- Manage and develop systems in the facility
- Complete all paperwork for charting as required by funding source
- Conduct groups periodically or as directed by management
- Housing Services:
- Provide Housing Assistance to clients
- Complete Housing Assessments and Referrals
- Complete HRA 2010-E's (CAPS)
Additional functions may include:
- Attend management trainings, team meetings, and staff meetings
- Provide assistance within the facility where needed
- Performs other duties as required by the Clinical Supervisor
Job requirements include:
- Educational/Training Required:
- Either CASAC or Bachelor degree preferred
- GED/High School Diploma or Associates Degree required
- CASAC-T required
- Experience Required/Language Preference:
- Minimum 2 years' experience with population in a residential setting or related field
- Computer Skills Required:
- Proficient in Microsoft Office and related programs
