Housing Outreach Liaison
This position is responsible for supporting the Residential Program by working in collaboration with direct service staff to resolve housing crisis issues. The expectation for this position is to improve client outcomes by increasing agency skills and knowledge pertinent to multiple barriers related to housing.
Duties and responsibilities include:
- Practice Housing First, harm reduction and other low-barrier service delivery models.
- Attend local Continuum of Care meetings related to coordinated entry/case conferencing.
- Outreach to individuals who are not currently engaged in CPR or CSTAR services who are in housing crisis or experiencing homelessness.
- Refer and transport (as needed) individuals to initial appointments to establish services for employment, behavioral health, substance use, physical health, or other mainstream benefits.
- Landlord recruitment and rapport building. Directly contact potential landlords and provide information and make referrals to their units. Follow up with landlords about referrals and problem solve tenant issues to maintain rapport.
- Receive referrals from Continuum of Care programs to assist individuals with housing stability.
- Obtain SSI/SSDI Outreach, Access, and Recovery (SOAR) certification and assist individuals throughout the SOAR process.
- Assist with the moving on process and meet with tenants to evaluate their readiness to move on to other permanent housing options.
- Stay current on new and improved institutional knowledge regarding housing resources
- Identify the network of peers and experts to assist with resolving housing related issues.
- Housing Outreach Liaison(s) shall attend all Housing Outreach Liaison conference calls, webinars, trainings, and meetings as arranged by the DBH or the Missouri Behavioral Health Council.
- Housing Outreach Liaisons shall provide training to internal agency staff and external community stakeholders about housing related best practices.
- The liaison shall track and provide all information in a timely manner as required by the designated staff of DBH and the Missouri Behavioral Health Council.
This job has no supervisory responsibilities.
Qualifications include:
- Bachelor's in Social Work, Psychology, Sociology or other related field and/or two years of relevant case management experience in housing. Communicate professional content orally and in writing; to maintain effective relationships with other staff disciplines and community agencies.
Certification or license required as required by the field.
