The Housing Case Manager works directly with families to assess their housing needs, develop individualized housing plans, and connect them to resources that support long-term stability. On a daily basis, this person conducts client intakes and assessments, provides ongoing case management, tracks participant progress, and coordinates referrals for services such as mental health support, substance use treatment, childcare, employment assistance, and domestic violence resources.
This role also plays a critical part in housing placement efforts by identifying available housing opportunities, communicating with landlords and property managers, assisting clients throughout the leasing process, and advocating on their behalf when barriers arise. The Housing Case Manager educates participants on tenant responsibilities, rental assistance programs, and budgeting strategies designed to promote self-sufficiency. Additionally, they work closely with landlords to resolve housing-related concerns, conduct housing inspections, perform rent calculations, maintain accurate documentation in case management systems, and participate in team meetings, case reviews, and community outreach efforts.

Required Skills & Experience
Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or a related field
2+ years of experience working with unhoused populations, housing programs, or community-based social services
Experience providing direct case management and developing individualized service plans
Knowledge of fair housing laws and housing placement practices
Understanding of Housing First, Motivational Interviewing, Harm Reduction, and Trauma-Informed Care models
Experience connecting clients to community resources and supportive services
Nice to Have Skills & Experience
Experience working with homeless families and rapid rehousing programs
Knowledge of child welfare systems and family reunification services
Experience building relationships with landlords, property managers, and housing providers
Previous experience negotiating housing placements, rent assistance, or lease agreements
Bilingual in English and Spanish
