What You’ll Do:
- In-home primary care for chronic disease management, dementia care and preventive healthcare
- Cognitive screenings, annual wellness visits, follow-ups and hospital-to-home transition supervision
What You Bring:
- Willingness to travel to patient’s homes and spend quality time with patients
- Active Virginia Nurse Practitioner
- DEA registration or eligibility
- Medicare PECOS enrolled
- Experience in primary care and geriatrics
