- Estimated Weekly Pay: $2,260 - $2,430
- Active Occupational Therapy certification/license
- At least 1 year of recent home health Occupational Therapy experience
- Experience using HCHB is required
- Strong clinical judgment, time management, and documentation skills
- Ability to work independently in a home health environment
- Comfort traveling within the assigned service area
- Provide skilled occupational therapy services to patients in the home setting
- Complete evaluations, treatment planning, and ongoing therapy visits
- Support patients in improving mobility, self-care, safety, and functional independence
- Maintain productivity goals while delivering high-quality patient care
- Complete documentation by the following morning deadline
- Collaborate with the care team to ensure coordinated, patient-centered outcomes
- Travel as needed throughout the assigned counties and service territory
