Hospital Clinical Social Worker – Behavioral Health & Discharge

The Union Clinical Social Worker/Behavioral Health Clinician provides Social Work services to the hospital's patients and their families to address psycho-social, environmental and behavioral factors which impact on the healthy functioning and/or recovery of the patients and families.

The incumbent functions as a member of the health care team to provide direct clinical social work services including assessment, discharge planning, and coordination of services.
