What You'll Do
As a Hospice Social Worker, you'll collaborate with physicians, nurses, therapists, chaplains, and volunteers to provide compassionate psychosocial support, advocate for patients and families, and help develop individualized plans of care throughout the hospice journey.
Key Responsibilities
- Conductcomprehensive bio-psychosocial assessments and ongoing reassessments
- Provideemotional support, education, and counseling patients and families
- Advocatefor patients while coordinating care with the interdisciplinary hospiceteam
- Participatein daily team huddles and interdisciplinary group (IDG) meetings
- Assistwith advance directives, POLST forms, medical power of attorney, andguardianship
- Connectpatients and families with community resources, transportation, respitecare, and long-term care services
- Helppatients and families access financial assistance programs, disabilitybenefits and insurance resources including VA, SSI, Medicare, Medicaid andPrivate Insurance options
- Providebereavement support and referrals for grief counseling
- Coordinatewith chaplains, healthcare facilities, and community partners to meetpatient needs
- Educateand guide patients and families in understanding medical information,treatment plans, and available care options to support informeddecision-making
- Setmeaningful goals, ensure adherence to care plans, and address barriersthat may impact appropriate treatment
- Promotehospice philosophy while maintaining professional growth and bestpractices
- Master'sdegree in Social Work (MSW) from a program accredited by the Council onSocial Work Education (CSWE)
- CurrentMontana social work license (LSW or LCSW) or actively working towardlicensure
- Minimumof one year of social work experience in a healthcare setting required
- Hospice,home health, palliative care, or community-based care experience preferred
- Knowledgeof chronic disease management, end-of-life care, and psychosocial supportservices preferred
- Strongcommunication, counseling, advocacy, and care coordination skills
- Validdriver's license and ability to travel within the service area
- Bepart of a compassionate interdisciplinary team dedicated topatient-centered care
- Buildmeaningful relationships with patients and families during a criticalstage of life
- Makea direct impact through advocacy, emotional support, and resourcecoordination
