You’ll join an interdisciplinary team to deliver clinical visits, psychosocial support, and seamless care coordination in case management, admissions, or resource roles.
You must hold a current RN license, be able to travel across the service area, and work well with patients, families, and staff. EMR proficiency and reliable transportation are required.
