Liberty Health seeks a compassionate Hospice RN to deliver high-quality, person-centered end-of-life care in home and facility settings. The Hospice RN completes comprehensive assessments, develops and updates individualized care plans, administers medications and treatments, and provides skilled symptom and pain management. This role coordinates closely with physicians, social workers, and other team members to support patients and families emotionally and clinically.
The nurse documents accurately in the EMR, educates caregivers on disease processes and care techniques, and participates in on-call rotation and quality improvement initiatives.
