Atlantic Visiting Nurse, part of Atlantic Health, is seeking an experienced Hospice Registered Nurse to join our interdisciplinary team and provide compassionate end‑of‑life care in patients' homes.
The RN will serve as primary case manager, conduct thorough clinical assessments, educate patients and families, ensure regulatory compliance, and collaborate with physicians and the IDT to deliver individualized care while maintaining autonomy in a home setting.
