Three Oaks Hospice is seeking a dedicated Registered Nurse to administer skilled nursing care for clients in their homes and coordinate with the interdisciplinary team. You will assess patient needs, implement and revise plans of care, and educate families on end-of-life care.
You will collaborate with referral sources, offer on-call coverage, and ensure high-quality, patient-centered care within a unified network of sister organizations. A valid RN license and CPR certification are required.
