You will steer an interdisciplinary group of RNs, Aides, Social Workers, and Chaplains, collaborating with the Program Director, Hospice Physician, and Community Liaisons to drive patient care, grow caseload, and ensure adherence to Federal, State, ACHC, and Crescent Hospice guidelines.
