About the Role
As a Hospice Registered Nurse Case Manager, you will provide skilled nursing care and compassionate support to patients and families during life's final chapter. You will coordinate comprehensive care for your caseload, collaborating with physicians, facilities, and interdisciplinary team members to ensure comfort, dignity, and quality of life.
Location
Office Location: Kalamazoo, MI.
Main Service Area:
- Kalamazoo / St. Joseph / Allegan
- Barry / Calhoun / Branch Counties
- Allegan / Van Buren / Berrien / Cass Counties
Hours
- Full Time: Monday to Friday, 8 a.m. to 5 p.m., with a weekend on‑call rotation
- Part Time: Three weekdays, 8 a.m. to 5 p.m., with a weekend on‑call rotation
- Per Diem: Flexible weekdays, Monday to Friday, 8 a.m. to 5 p.m., with a weekend on‑call rotation
Required Qualifications
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
- Current State License as a Registered Nurse
- Minimum one (1) year experience as a nurse in a clinical care setting
- Reliable transportation, valid driver’s license, and valid automobile insurance coverage
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- Hospice or home health experience
Essential Skills and Competencies
- Nursing skills as defined by accepted nursing standards
- Ability to assess and respond to the needs of patients, families, and caregivers in varied settings and effectively function in home or nursing facility environments
- Skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles
- Ability to handle multiple priorities, documentation requirements, and deadlines
Pay
$80,000—$85,000 USD
