About the Role

As a Hospice RN Case Manager, you will provide skilled nursing care and compassionate support to patients and families during life's final chapter, ensuring comfort, dignity, and quality of life. This role offers the full scope of nursing practice and the autonomy to coordinate comprehensive care while collaborating with an interdisciplinary team to address physical, emotional, and spiritual needs.

Location

  • Office Location: Grand Rapids, MI
  • Main Service Area Options:
    • Kent County
    • Muskegon / Ottawa Counties
    • Newaygo / North Kent County
    • Ionia / Montcalm / Mecosta Counties
    • Oceana County
  • This role requires high travel and daily travel within the service area to provide care in patients' homes, assisted living facilities, or nursing homes.

Hours

  • Full Time: Monday to Friday, 8 a.m. to 5 p.m., with a weekend on‑call rotation
  • Part Time: Three weekdays, 8 a.m. to 5 p.m., with a weekend on‑call rotation
  • Per Diem: Flexible weekdays, Monday to Friday, 8 a.m. to 5 p.m., with a weekend on‑call rotation

Required Qualification

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current State License as a Registered Nurse
  • Minimum one (1) year experience as a nurse in a clinical care setting
  • Reliable transportation, valid driver’s license, and valid automobile insurance coverage

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN)
  • Hospice or home health experience

Essential Skills and Competencies

  • Nursing skills as defined by accepted nursing standards
  • Ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environment
  • Skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles
  • Ability to handle multiple priorities, documentation requirements, and deadlines

Pay Range

$80,000 - $85,000 USD
