Well Care Health is seeking a Hospice Registered Nurse Case Manager to join our Advance, NC team. Provide skilled nursing for hospice patients, manage caseloads, and educate families about end-of-life care within an interdisciplinary team.
The RN will assess needs, develop individualized plans of care, coordinate with physicians, and offer emotional support to families while upholding our mission and values.
