As a Hospice RN, you’ll act as a trusted Case Manager, delivering care that supports patients in their homes and communities. You will guide families through care decisions, while providing comfort, dignity, and compassionate support during life’s most tender moments.

You will:Complete comprehensive assessments of patients and families to determine hospice needsDevelop and implement individualized care plans, revising as neededProvide skilled nursing care, administer medications, and manage symptomsEducate patients and families on care techniques, medications, and comfort measuresMaintain accurate clinical documentation and communicate changes to physiciansParticipate in interdisciplinary team meetings and quality improvement initiativesProvide emotional support and counseling to patients and familiesSupervise hospice aides and delegate responsibilities appropriatelyCollaborate with an interdisciplinary team that shares your commitment to excellenceParticipate in on-call rotation as neededWhat We OfferCompetitive pay and benefits, with flexible schedules to fit your life and support balanceOngoing professional development and trainingA truly collaborative team that celebrates your successWhat We’re Looking ForCurrent RN license in the State of ArizonaMinimum of 2 years of nursing experience, hospice or home health experience preferredHomecare Homebase (HCHB) experience preferred, not requiredValid driver’s license and reliable, insured vehicleStrong clinical skills and a passion for patient-centered careAbility to work independently and a passion for making a real differenceThe employer for this position is stated in the job posting.
