Sanctuary Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) for per diem coverage to support patients and families with high-quality end-of-life care in Summit, Portage, Medina, Stark and Wayne County.
In this role, you will collaborate with our Clinical Director, respond to referrals, and provide direct nursing care with a focus on comfort and quality of life. A current RN license and at least 1 year of medical-surgical/acute care experience are required.
