Life Source Hospice is seeking a compassionate and experienced Registered Nurse Case Manager to join our interdisciplinary hospice team. The RN Case Manager is responsible for coordinating and providing individualized, patient-centered care to hospice patients and their families in accordance with the plan of care, physician orders, organizational policies, and applicable state and federal regulations.
Schedule
Full-time position
Monday through Friday, with flexibility based on patient needs
Participation in the on-call rotation for one weekend per month
Rotating holiday on-call coverage required
Responsibilities
Conduct initial and ongoing comprehensive nursing assessments
Develop, implement, and update individualized plans of care in collaboration with the patient, family, attending physician, Hospice Medical Director, and interdisciplinary team
Provide skilled nursing care focused on symptom management, pain control, comfort, and quality of life
Educate patients, families, and caregivers regarding disease progression, medications, symptom management, safety, and end-of-life care
Coordinate medications, medical supplies, durable medical equipment, and other services related to the terminal diagnosis
Communicate changes in the patient’s condition promptly to the appropriate physician and interdisciplinary team members
Make routine, as-needed, and crisis visits based on patient needs
Participate in interdisciplinary group meetings and contribute to ongoing care planning
Maintain timely, accurate, and complete clinical documentation in the electronic medical record
Ensure care is delivered in compliance with CMS Conditions of Participation, state regulations, and organizational policies
Support patients and families through the physical, emotional, psychosocial, and spiritual aspects of end-of-life care
Provide appropriate handoff communication for on-call coverage
Participate in quality improvement, infection control, and staff education activities as assigned
Perform pronouncements and post-mortem care coordination when required
Demonstrate sensitivity and respect for each patient’s values, preferences, culture, and end-of-life wishes
Qualifications
Current, unrestricted Registered Nurse license in the State of New Jersey
Graduate of an accredited school of nursing
Minimum of one year of professional nursing experience required
Hospice, home health, palliative care, oncology, geriatrics, or acute-care experience strongly preferred
Current CPR certification
Strong clinical assessment, symptom-management, communication, and organizational skills
Ability to work independently in patients’ homes while collaborating effectively with an interdisciplinary team
Ability to manage multiple patients and changing clinical priorities
Valid driver’s license, reliable transportation, and automobile insurance
Ability to participate in weekend and holiday on-call rotations
Proficiency with electronic medical records and basic computer applications
