Overview
Hospice RN Case Manager / Registered Nurse – HealthCare Support is seeking a Hospice RN Case Manager / Registered Nurse to fill an opening with a prestigious Hospice organization in Tennessee. Location: Dickson / West Nashville area.
Responsibilities
- Conduct home visits or facility visits to provide patient care.
- Collaborate with an NP, RN team, social worker, and other case managers to promote ongoing care.
- Provide wound care, monitor vitals, administer medications, and perform pronouncements of death as needed.
Schedule
Monday – Friday, 8:00am – 5:00pm. On-call one night per week from 5:00pm–11:00pm (no weekends).
Qualifications
- Active RN License in Tennessee
- At least 2 years of RN experience
- Hospice or Palliative care experience
- Related experience in Hospice or Home Health or Oncology or ICU or Med/Surg or LTC/SNF

Pay Details
Pay: $74,173 - $84,886 per year (based on experience) + bonus eligibility
