Brookhaven Hospice is seeking a Full Time Hospice RN Case Manager to join our dedicated nursing team providing exceptional end-of-life care in the community. You will deliver hands-on nursing care, coordinate services, and guide patients and families through the hospice journey with compassion.
The role requires an RN with at least 1 year of medical-surgical/acute care experience (hospice a plus). Proficiency in EMR and a valid driver's license are required.
