nHospice Case Managers are vital members of our care team, providing compassionate, skilled nursing support to patients and families during end-of-life care. As a Hospice RN Case Manager, you will oversee patient care, coordinate interdisciplinary support, and ensure comfort, dignity, and quality of life for every patient you serve.

We are looking for an RN Case Manager who understands that they are the "Captain of the Ship" for every patient under their care. nThe Role: Total Case Management
nIn this position, you are not just making visits—you are managing the entire clinical ecosystem of your patients. You are the primary point of contact for the family and the central hub for the Interdisciplinary Group (IDG).
nYour Responsibilities as Case Manager:
nThe Clinical Director of Your Team: You lead and coordinate the care provided by your MSW, Spiritual Counselor (SC), and CHHA. You are responsible for ensuring their interventions align with your overall clinical strategy.
nDirect Physician Collaboration: You work hand-in-hand with the MD. You don’t just report symptoms; you provide clinical assessments, suggest interventions, and secure medication orders and treatment plans immediately.
nComprehensive Assessment: Perform full-body physical and psychosocial assessments at every visit. You are responsible for identifying changes in status before they become crises.
nThe Plan of Care (POC): You own the POC. You develop, update, and maintain a compliant, individualized plan that reflects the patient’s goals and regulatory requirements.
nMedication Management: You are responsible for the entire medication profile, including reconciliation, pharmacy coordination, and symptom management (GIP/Continuous Care triggers).
nIDG Leadership: You lead the discussion for your patients during IDG meetings, providing the team with clear, concise, and professional updates that meet all CMS and Title 22 standards.
nResponsibilities
nComplete comprehensive nursing assessments and develop individualized plans of care in collaboration with patients and families.
nProvide direct hospice nursing care to maintain the highest level of comfort and quality of life.
nManage and update care plans as patient needs change.
nCommunicate with physicians to obtain appropriate orders and report changes in patient condition.
nInitiate interventions and evaluate outcomes to ensure optimal symptom management.
nEducate patients, families, and caregivers about hospice care, medications, and symptom recognition.
nAccurately document patient care and communicate updates to the interdisciplinary team (IDG).
nCoordinate with external service providers as needed to meet patient and family needs.
nEnsure availability of necessary medical equipment and supplies.
nAssign and supervise Home Health Aides, completing supervisory visits every 14 days.
nServe as IDG coordinator, collaborating with all team members to ensure holistic care.
nParticipate in on-call rotation to provide 24/7 hospice coverage and support.
nQualifications
nGraduate of an accredited school of nursing.
nCurrent California RN license (BSN preferred).
nCertification in Hospice and Palliative Nursing desired.
nMinimum of one year of professional nursing experience within the last three years, or a BSN degree.
nStrong clinical judgment, communication, and documentation skills.
nCompassionate and supportive attitude toward patients and families facing end-of-life care.
nAbility to maintain patient confidentiality and professionalism at all times.
nProficiency in computer use and electronic medical record (EMR) documentation.
nReliable, well-maintained personal vehicle with valid California driver’s license, acceptable driving record, and proof of auto insurance.
nSpanish speaking a plus!
nLicensure: Current, unencumbered California Registered Nurse (RN) license.
nHospice Mastery: Minimum 1–2 years of Hospice Case Management experience is required. You must know the "Hospice Benefit" inside and out.
nRegulatory Knowledge: Deep understanding of CMS Conditions of Participation (CoPs) and California Title 22. Or willingness to learn!
nTriage Skills: Expert ability to prioritize acuity and manage a field schedule effectively.
nDocumentation Excellence: Proficiency in EMR (e.g., HospiceMD, KanTime, MatrixCare) with an emphasis on "gold-standard" clinical narratives.
nCommunication: Must be able to speak professionally with physicians and compassionately with families.
nSchedule & Compensation
nFull-Time: Monday to Friday, 8:30 AM – 5:00 PM
nOn-Call: 2 weekend shifts per month (as needed)
nHolidays: 2 shifts per year (as needed)
nPay Range: $50 – $54 per hour (based on experience)

nField-based: Serving patients across Orange County and the Greater Los Angeles areas
