Requirements:

  • Current RN license in California
  • One year of experience preferred
  • Minimum one year of case management experience preferred
  • Experience in hospice preferred

Duties include:

  • Ensuring quality and safe delivery of hospice services
  • Providing direct hands-on care within scope of practice
  • Obtaining orders to promote comfort and quality of life
  • Keeping the Case Manager informed of patient condition changes
  • Attending daily patient review meetings and bi-monthly interdisciplinary meetings
