AccentCare is seeking a compassionate Hospice RN Case Manager to support patients receiving end-of-life care. In partnership with Hanford Pharmaceuticals, you’ll manage a panel of hospice patients, perform comprehensive assessments, and develop individualized care plans that align with patient and family goals. You will oversee pain and symptom management, administer medications, and coordinate services with physicians, pharmacists, and the interdisciplinary team.
