About This Role
As the Hospice Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient, continuously evaluating performance and collaborating with other professionals to implement treatment strategies that promote well-being.

What You'll Do
- Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
- Identify the patient's/family's physical, psychosocial, and environmental needs, reassessing no less than every 15 days.
- Initiate and coordinate the plan of care.
- Document problems, appropriate goals, interventions, and patient/family responses to hospice care.
- Collaborate with the patient/family, attending physician, and other members of the IDG in providing daily care.
- Instruct and supervise the patient/family in self-care techniques when appropriate.
- Maintain accurate and relevant clinical notes regarding the patient's condition.
Requirements
Education:
- Current and unrestricted RN licensure in the state of practice
Experience:
- 1+ years of clinical experience preferred
Licenses & Certifications:
- Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation
- Current CPR certification or ability to complete within 90 days of hire preferred
