Apreva Hospice is seeking an RN Case Manager to provide 1:1 patient care in a Central / Coastal San Diego setting. You will assess needs, develop care plans, administer medications and manage symptoms, while educating families and coordinating with the interdisciplinary team.
Office location is San Diego with on‑site hours Mon-Fri 8:00 AM - 5:00 PM. Prior hospice experience and a valid California nursing license are required or preferred.
