Hospice RN Case Manager, South Dallas Territory
Location: Dallas, TX (Field-based, South Dallas territory)
Job Type: Full-time
Pay: $85,000 to $95,000 per year
Guide patients and families through end-of-life care with compassion, clinical skill, and the support of a strong interdisciplinary team.
Must-Haves
- Active RN license valid for practice in Texas
- 2+ years of RN experience in a clinical care setting
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto liability insurance
- Ability to work independently and make sound clinical decisions
- Strong written, verbal, organizational, and interpersonal skills
What You’ll Do
- Coordinate and oversee all aspects of assigned patients’ hospice care
- Develop and implement individualized plans of care with the interdisciplinary team
- Perform head-to-toe assessments, monitor symptoms, and manage medications
- Provide symptom management and comfort-focused care
- Educate patients and families on condition, needs, and care
- Communicate progress with physicians and clinical leadership
- Complete accurate, timely documentation
Nice-to-Haves
- 1+ year of full-time hospice experience
- Home health or palliative care experience
- CHPN certification
