Heartland Hospice seeks an experienced Hospice RN / RN Case Manager to lead patient-centered care in patients' homes across Michigan. You will conduct comprehensive assessments, implement and adjust plans of care, and collaborate with physicians, social workers, chaplains, and aides to ensure compassionate end-of-life support.
You will educate families, manage symptoms, document care meticulously, and participate in on-call rotations as required by the program.
