Badger Hospice, LLC is seeking a compassionate Hospice RN Case Manager to provide skilled, comfort-focused care to patients and families during the end-of-life journey. You'll coordinate care, manage symptoms, and collaborate with our interdisciplinary team to ensure every patient receives exceptional, personalized hospice care.

What You'll Do
- Complete patient assessments and develop individualized care plans.
- Coordinate care with the interdisciplinary hospice team.
- Provide skilled nursing care, symptom management, and family education.
- Monitor patient needs and maintain accurate documentation.
What We're Looking For
- Current Wisconsin RN license in good standing.
- Hospice, palliative care, or case management experience preferred.
- Strong assessment, communication, and organizational skills.
- Valid driver's license, reliable vehicle, and ability to travel.
