Job description

Vantage Hospice is expanding into the Bryan/College Station area! We are growing and seeking an experienced, compassionate RN Case Manager to join our team.

Position Reports To: Clinical Manager

Job Type: Full-Time

Qualifications

  • Current licensure/registration as a Registered Nurse (RN) in the State of Texas.
  • Previous hospice nursing experience preferred.
  • Experience caring for terminally ill patients and families with strong knowledge of symptom control and emotional support.
  • Background in Oncology, Home Health, or Long-Term Care is a plus.
  • Excellent physical and psychosocial assessment skills with strong reasoning, deduction, and critical thinking abilities. Must demonstrate independence in clinical assessment.
  • Ability to prioritize urgent patient, family, and facility needs while managing multiple tasks simultaneously.
  • Strong time-management and organizational skills with flexibility in scheduling.
  • Must display a professional attitude and demeanor at all times.
  • Reliable transportation, valid Texas driver’s license, and state-required liability insurance (or access to reliable transportation) required.

Responsibilities

  • Conduct comprehensive patient assessments and develop individualized hospice care plans.
  • Coordinate and manage patient care services while ensuring compliance with HIPAA and all regulatory guidelines.
  • Collaborate with the interdisciplinary team to support discharge planning and post-acute care needs.
  • Educate and support patients and families regarding hospice services and end-of-life care.
  • Maintain accurate, timely documentation of patient progress and care interventions in accordance with agency policies.

Education

  • Associate Degree in Nursing (Required)
