Position Summary

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence coordinate care with the interdisciplinary team patientfamily and referring agency; and assumes the responsibility for coordination of care

Essential Duties

Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical functional psychosocial and environmental needs and ADLs ie risk for grief culturalspiritual verbal non verbalImplement the individualized POC and recommend revisions to the plan as necessaryConsulting with and educating the patientfamily regarding disease process self care techniques end of life care nutrition and dietary needs Providing training to other staff as neededInitiating appropriate preventive and rehabilitative nursing proceduresPreparing clinical and progress notes that demonstrate progress toward established goalsCoordinating all patientfamily services and prioritization of needs with the members of the IDTUse of case management approach and referring to other services as neededInforming physician and other personnel of changes in the patients needs and outcomes of intervention while evaluating patientfamily response to careDetermining scope and frequency of services needed based on acuity and patientfamily needsAssessing ability of the caregiver to meet the patients immediate needs upon admission and throughout careEvaluating own needs for support and using identified systems to meet the needCommunicating information using current process and technology available to the organizationApplying specific criteria for admission and re certification to hospice care to establish appropriate levels of care and the patients eligibilityPerform on call responsibilities and provide on call service to patientsfamilies as assignedOther duties as assigned

Qualifications

Graduate of an accredited Associate or bachelors degree in NursingCurrent state license as an RN and current state drivers licenseTwo years experience as an RN in a clinical care setting; previous hospice or home health experience preferredCurrent CPR certificationProficient skill using a computer is required; Microsoft office suite experience is preferredStrong interpersonal skills and ability to provide effective support to the patient and familyDemonstrate skill in nursing practice ability to cope with family emotional stress and tolerance of individual lifestylesStrong communication and organizational skills and high standard of ethics and compliance
